b.W/ TIN # Change RDO: use 1905 BIR form to change RDO employer

3. SSS No. or SSS ID CARD

a. First time applicants: use E1

b. Change of status: use E4

c. W/ Loan: Submit updated SSS Loan Statement of Account

4. PHIL HEALTH NO. (MDR)

5. PAG-IBIG (MDF, Loyalty Card)

a. W/ Loan: Submit updated Pagibig Salary Loan Statement of Account

6. NBI (Valid & Original)

7. ATM form Requirements for ATM at least * one (1) digitized ID from the ff: *UMID *SSS ID *Loyalty Card (PAG-IBIG) *Driver’s License *Passport *NBI

8. Health Clearance

8.1Medical Examination

a. X-ray d. Drug test

b. Urinalysis e. Fecalysis

c. Complete Blood Count f. Certificate of fit to work

8.2Health Certificate/ Health Card (LGU issued based on place of work)

8.3Working Permit/ Occupational Permit (LGU issued based on place of work)
